CSCI New Employee Vaccination Attestation

CSCI has partnered with Privia Health to gather information around employee vaccination status and collect copies of employee vaccination records. CSCI will not be maintaining records of the cards as all records will be housed by Privia Health.

To attest to your COVID-19 vaccination status you will need to answer a short survey and upload a photograph of your vaccination card.
